Rochdale Health Leaders Present Collaborative Care Model at European Conference

Health leaders from Rochdale, Greater Manchester, presented their collaborative working model at the International Conference on Integrated Care held in Antwerp, Belgium. The event attracted over 1,000 delegates, where they shared insights on improving health and social care services.

Dr Salman Shahid, Associate Medical Director of the Greater Manchester Integrated Care Board, led the presentation alongside a team from One Rochdale Health and Care. They focused on the 'Integration Continuum' and its role in achieving patient-centred care within an integrated healthcare framework.

During the conference, Dr Shahid emphasised the need for a multi-disciplinary approach to tackle health inequalities in Rochdale, which is one of ten areas in Greater Manchester facing significant challenges. He noted the importance of collaboration among hospitals, community services, GP practices, and voluntary organisations to enhance patient and public involvement in health services.

Dr Shahid, along with colleagues Dr Kirsty Marshall, Dr Shona McCallum, and Alison Kelly, highlighted their work on improving patient engagement in health and social care. They discussed how their integrated approach aims to create a seamless health and well-being model with shared responsibility for patient care.

The conference provided an opportunity for Rochdale's health leaders to learn from colleagues across Europe, aiming to refine their approaches and ultimately improve the health and well-being of residents in the region. Dr Shahid stated that the insights gained would help enhance their systems and create a better health and social care framework for local residents.
